Below are some forms and guidelines for new clients.  If you have access to a printer, it will speed up the process of registering to print and bring the necessary completed forms with you for your first visit.  If you are unable to print them ahead of time, DON'T WORRY, the process of registering is easy and we are always happy to provide the forms when you arrive!
